The Delta Dental Mobile Program will be rolling into Yankton, June 3-7, 2024 to once again provide dental care to youth ages 0-21, residing in South Dakota, at no cost to the child or family.

Delta Dental of South Dakota supports the Mobile Program with paid staff and volunteer dental professionals. The mobile dental bus is on the road approximately 40 weeks a year, serving an estimated 1,300 children annually in South Dakota.

The cost to bring this program to Yankton is $2,500.00 – and sponsored by United Way. Another great example of how your contribution to United Way of Greater Yankton provides critical services to the community!

 

HOW DO YOU or YOUR CHILD PARTICIPATE?

 1) Pre-Registration is required. All patients must be ages 0-21, residing in South Dakota.

     

You may click on the registration button(s) below to obtain, complete and print the registration form.

If printing the form is not an option for you, blank registration forms can be obtained at Yankton School District, Boys & Girls Club of Yankton, Department of Social Services, Yankton Contact Center, The Clothing Closet, and United Way of Greater Yankton.

2) Submit your Registration Form.

 

All completed registration forms must be submitted to United Way of Greater Yankton at 920 Broadway Avenue, Yankton, SD. by Thursday, May 30, 2023.

3) Once a Registration Form is received, United Way Staff will schedule you or your child's first dental screening with Delta Dental.
